COVID-19 Vaccines will be available at JCU Health from 24 March 2021 as part of the national vaccine rollout.

If you do not meet the eligibility criteria just yet, please wait and call us when advised as per the eligibility and keep an eye on the website and Facebook page for further updates.

If you have an appointment with us for the COVID-19 vaccine, please complete the form below and email it to [email protected] or bring it in on the day of your appointment. If you are unable to do this, please attend your appointment 15 minutes early to allow time to complete the paperwork prior to your scheduled time.
